New Patient Form!
At this time, Physicians at our Clinic are accepting new patients. You can complete the New Patient Intake Form, to the best of your knowledge. Once complete, please either drop it off at the clinic or you may email it to info.southtownmed@gmail.com.
The Physicians and office staff will review the New Patient Intake Form, assign a Physician depending on preference, and you will be contacted to schedule a Meet and Greet appointment. Please note that Meet and Greet appointments take a minimum of 30 minutes so please plan accordingly.

Uninsured Services are services provided that are not insured by the Manitoba health insurance plan, such as medical reports requested by insurance companies, medico-legal reports, transfer and access of medical records, sick notes, and driver’s medicals.
They may also include services provided to individuals who are not insured, such as those who are not Canadian residents.
